WHO CAN REQUEST A CERTIFIED COPY OF A VITAL RECORD
(Pursuant to State of New Jersey Executive Order #18)


A State or local Registrar may issue a certified copy of a vital record only to persons who establish themselves as the subject of the vital record, the subject's parent, legal guardian or legal representative, spouse, child, grandchild or sibling, if of legal age, to a State or federal agency for official purposes, pursuant to court order or under other emergent circumstances as determined by the Commissioner.

WHAT IS REQUIRED TO OBTAIN A CERTIFIED COPY OF A VITAL RECORD

 

  1. The exact name(s) that is currently recorded on the birth, marriage, civil union, domestic partner or death record (first-middle-last)
  2. The exact place of birth, marriage, civil union, domestic partner or death (city)
  3. The exact date of birth, marriage, civil union, domestic partner or death (month-day-year)
  4. The mother's maiden name
  5. The father's name (when recorded)
  6. A Valid Photo ID with current address on it

 

WHAT IS ACCEPTABLE IDENTIFICATION


Acceptable Identification is:

Valid Photo ID with current address on it,(not a P.O. Box)
Or
Valid Photo ID without address and One (1) additional form of ID with current street address
Or
Two (2) Valid alternate forms of ID with current street address, such as non-photo driver’s license, vehicle registration, insurance card voter’s registration, passport, Green card, County ID, Utility bill(s), or other as approved by STATE REGISTRAR OR ASSISTANT STATE REGISTRAR.

Records are kept in the town where the event occurred, so certificates for birth, deaths, domestic partnership or marriage/civil unions which occurred in Toms River Township ONLY can be provided at this location.


CERTIFIED COPY OF A BIRTH CERTIFICATE

 

To order a copy of a birth certificate you must provide:

A fee of $10.00 per certified copy

The exact name of the child at time of birth (first-middle-last)

The mother's maiden name

To order a copy of a death certificate you must provide:

A fee of $10.00 per certified copy

The exact name of the decedent at time of death (first-middle-last)

The exact place of the death (city) (We will only have it if death occurred in Toms River)

The exact date of the death (month-day-year)

The relationship to decedent

The decedent’s mother’s maiden name

The decedent’s father’s name (when recorded)

A Valid Photo ID with current street address on it (not a P.O. Box)

Depending on the relationship to the decedent, other ID will be asked for to show relationship (your birth certificate with parent’s names on it, your marriage license, or other items to connect you to the decedent)

The exact name of the Bride/Applicant “B” (first-middle-last)

The exact place of the marriage/civil union (city) (We will only have it if marriage/civil union occurred in Toms River)

The exact date of the marriage/civil union (month-day-year)

A Valid Photo ID with current street address on it (not a P.O. Box)

Depending on the relationship to the couple, other ID will be asked for to show relationship (your birth certificate with parent’s names on it, your marriage license, or other items to connect you to the decedent)

REQUIREMENTS FOR A MARRIAGE LICENSE / CIVIL UNION

1) File for the license in the town where the APPLICANT is a resident.

a) If the applicants are not a resident of the state of New Jersey, you would then file in the town where the marriage/civil union is taking place. The license is then only good in that town.

b) Applicants and Witness need Valid Photo ID with current address on it, (not a P.O. Box) to show where they are residents.

 

2) There is a 72 hour waiting period, from the time you make application for the marriage license/civil union until the time you can pick up the license. You do not have to pick up the license in 72 hours. You may leave it here until it is closer to your wedding date, but you are eligible to pick it up in 72 hours.

3) In order to apply the applicants and one witness over the age of 18 (that knows both of you) must appear before the Registrar.

 

4) We require Birth Certificates for proof of age. If you do not have your Birth Certificate, TWO other forms of ID with your date of birth on them are required. Other acceptable forms are:

 

Driver’s License
Passport
Baptism Paper

5) If either one of you or both are divorced or have dissolved a civil union, we need to see the divorce papers or the dissolution of the civil union, with the FILED DATE AND DOCKET # on them. It must be the FINAL DECREE signed by the JUDGE

 

6) If either one of you are a widow or widower we need to see the Death Certificate.

 

7) Any person UNDER THE AGE OF 18 needs parental consent papers signed by both parents. If under the age of 16 the parental consent must be signed by both parents and judge.

8) No license will be released from this office without the following information:

The intended date of marriage/civil union
The name and address of the person performing the marriage/civil union
The place where the marriage/civil union is to take place

9) The cost of the marriage license is $28.00.
